LifePath is hiring a full-time Geriatric Support Services and Long-Term Care Services Coordinator (SCO/OC Case Manager) based in its Greenfield office, serving Franklin County and the North Quabbin area. The role conducts person-centered assessments, develops and monitors service plans, authorizes internally funded services, makes referrals, coordinates with providers, advocates for consumers, and maintains records. It requires a bachelor’s degree preferred and 2–3 years of health or human services experience; a Social Work license is a plus. The position offers medical, dental, and vision insurance, PTO, a 401(k), and a 6% pay differential for applicable bilingual skills.
