The Geriatric Wellness Coordinator provides on-site behavioral health, social service, case management, counseling, crisis intervention, and wellness programming for adults and seniors in supportive housing. The role assesses residents, develops service plans, coordinates care with families and service providers, supports housing stability, and works with property management and maintenance staff.
