Description
The position provides direct case management and person-centered planning services to individuals served by St. Clair County Community Mental Health, including needs assessment, service coordination, goal development, monitoring, community resource linking, and recordkeeping. It requires a bachelor’s degree in a relevant field, a valid driver’s license, at least one year of related experience, experience with SMI and IDD populations, Microsoft Office and Windows 10 proficiency, transportation access, and QMHP, QIDP, or CMHP qualification. The role has no supervisory responsibilities and requires light physical activity, occasional lifting of up to 25 pounds, and compliance with background checks and drug screening.
