Description
ADAPT Community Network, formerly United Cerebral Palsy (UCP) of NYC, seeks a Habilitation Specialist to design, implement, and monitor individualized habilitation and skill-building programs for individuals with developmental disabilities. The specialist will lead a team of Habilitation Assistants, coordinate service delivery, and ensure person-centered support aligned with ADAPT's policies and OPWDD regulations. Responsibilities include developing and monitoring treatment plans, providing supervision and training for assistants, liaising with families and clinicians, documenting progress, utilizing community resources, and assisting participants with daily living activities.
