Project Renewal is hiring a Harm Reduction Case Manager for its In Homes Now program, which provides permanent housing and case management to formerly homeless individuals. The role manages a caseload of 16 to 18 clients, conducts home visits, assists with medical, dental, and substance abuse referrals, maintains clinical documentation, develops service plans, and coordinates with interdisciplinary staff. A bachelor’s degree is preferred, while experience working with homeless or substance-affected individuals and strong writing and time-management skills are required; Spanish fluency is a plus. The salary range is $44,000 to $48,000.
