CAMBA is hiring a full-time Homes Plus Coordinator in Brooklyn to provide intensive case management and care coordination for adults with serious mental illness enrolled in a Health Home. The role carries up to 20 clients, conducts in-person and telephone visits, develops care plans, coordinates referrals and appointments, advocates for clients, assists with benefits applications, and documents progress. Candidates need a qualifying degree and experience or a CASAC credential with experience, strong communication and written skills, Microsoft Office proficiency, and the ability to work with reporting platforms; bi-lingual English and Spanish is preferred. Compensation is $64,519 annually, with health and dental insurance and other benefits.
