JFCS is hiring a High Needs Case Manager in Phoenix to provide case management and ongoing support to children receiving behavioral-health services and their families or caregivers. The role includes weekly client contact, hospital discharge coordination, service referrals, prior authorizations, agency collaboration, and facilitation of Child & Family Team meetings. Applicants must meet one of three education and behavioral-health experience requirements, hold a Level 1 fingerprint clearance, meet agency MVR requirements, and have reliable transportation, an Arizona driver’s license, and Arizona auto insurance.
