Description
The HIV Care Manager will oversee Ryan White and Philadelphia Division of HIV Health program performance, staffing, funding, compliance, reporting, quality reviews, and interdisciplinary coordination across PHMC Health. The role supervises HIV Medical Case Managers and PrEP Patient Navigators, serves as a liaison to city, state, nonprofit, pharmaceutical, and community partners, and supports staff training, documentation, performance monitoring, and process improvement. It requires two years of case management or care coordination experience, supervisory experience preferred, and a master’s degree or bachelor’s degree in a related field with three years of infectious-disease, HIV case management, PrEP, or care coordination experience.
