NAFI Connecticut is hiring a Home Based Case Manager to provide in-home case management services to adolescents who are removed from or at risk of removal from their homes due to high-risk behaviors. The role works with a clinician on a caseload of 15 clients and families, conducts weekly home visits, supports parenting-group facilitation, participates in training, and delivers services with fidelity to the Parenting with Love and Limits model. The position is statewide, based in Warwick, requires a flexible schedule with non-traditional hours, and offers a salary of $55,000 per year plus medical, dental, prescription, and vision coverage and other benefits.
