CenterWell Home Health is hiring a Home Health LPN to provide skilled, one-on-one nursing care in patients’ homes under the direction of an RN. The role includes implementing care plans, educating patients and families, documenting care, coordinating with physicians and other clinicians, and participating in discharge planning. Applicants need a current nursing license, valid driver’s license, CPR certification, and two years of LPN/LVN clinical experience; home health or hospice experience is preferred. The position is scheduled for 40 hours per week and offers a starting base pay of $57,200 to $78,800 per year, with medical, dental, vision, retirement, and other benefits.
