CenterWell Home Health is hiring a Home Health LPN to provide skilled, one-on-one nursing care in patients’ homes under the direction of an RN. The role includes implementing care plans, educating patients and families, documenting visits and interventions, coordinating with physicians and other clinicians, and participating in discharge planning. Applicants need a current nursing license, valid driver’s license, CPR certification, and two years of LPN/LVN clinical experience; home health or hospice experience is preferred. Compensation is $33.00–$47.00 per visit or unit, with a stated annual range of $53,000–$73,000 for full-time employment.
