Description
The Home Visiting Program Coordinator supports the day-to-day operations of a home-visiting program by serving as a patient and family contact, conducting initial triage, scheduling in-home visits, maintaining medical records, processing medication and durable medical equipment requests, coordinating care transitions, and routing faxes and documentation. The role is hybrid, with most responsibilities performed remotely and site attendance expected when necessary. A high school diploma or GED is required, along with 2–4 years of clinical or healthcare administrative and customer-service experience, care-coordination experience, and proficiency with an EMR and Microsoft Office; an associate degree, medical billing experience, and Spanish fluency are preferred.
