Elara Caring is hiring a Hospice Certified Nurse Assistant/Hospice Aide to provide skilled nursing care in home health settings. The role involves following individualized plans of care and physician orders, monitoring patients, collaborating with the RN Case Manager, educating patients and caregivers, documenting medication regimens, and preventing avoidable hospitalizations. Applicants need a high school diploma or GED, one year of hospice aide or nursing assistant experience, hospice aide training meeting 42 CFR §484.36 requirements, and the ability to lift up to 50 pounds; home health experience is preferred.
