Description
Sutter Health is seeking a Hospice Nurse Practitioner to join their team. This independent advanced nursing professional will assess patient needs, create and modify care plans, make referrals, educate patients and families, monitor patient status, and evaluate the effectiveness of care. They will serve as a resource for medical and nursing staff, collaborate on Hospice programs, and provide evidence-based physical, emotional, psychosocial, and spiritual care to individuals and families facing life-limiting illnesses. The goal is to enhance the patient's quality of life during the illness trajectory, through death, and into the bereavement period.
