Ohio’s Hospice is hiring a PRN traveling Hospice Patient Care Specialist to provide compassionate, field-based care to patients and families in Ohio’s Hospice South region, including the greater Dayton area and surrounding counties. The role involves traveling to patient homes and care facilities, documenting care in electronic medical records, and working with an interdisciplinary team. It is a driving position covering varied day-shift hours and requires a high school diploma or GED, computer proficiency, a valid Ohio driver’s license, auto insurance, and the ability to pass background and drug screening; CHPNA certification is preferred but internal training is offered.
