Description
The Hospice RN provides hospice and palliative care in the home, community, or agency setting, including admission, routine, recertification, discharge, and death visits. The role follows the patient plan of care, performs nursing assessments, provides treatments and education, documents visits in the EMR, and updates the interdisciplinary team. Required qualifications include a state-approved nursing education, one year of nursing experience, an active RN license, CPR certification, and a valid driver’s license; CHPN certification is preferred.
