Description
Ohio's Hospice is hiring a field-based Hospice RN Care Manager in Marysville, Ohio, serving patients in homes and care facilities throughout the Marysville, Plain City, London, Jefferson, and surrounding communities. The role provides compassionate nursing care, assesses patients and families, develops and adjusts care plans, coordinates with the interdisciplinary hospice team, educates caregivers, advocates for patients, documents in the electronic health record, and participates in admissions, care conferences, and quality improvement activities. The schedule is Monday through Friday, 8:00 AM to 4:30 PM, with holiday and weekend rotation. An accredited nursing program, current Ohio RN license, CPR certification, valid driver's license, automobile insurance, territory travel, and background and drug screening are required; acute care, hospice or palliative care, and CHPN experience are preferred.
