Interim HealthCare is hiring a Hospice RN Case Manager in Battle Creek, Michigan, to provide home-based skilled nursing care to patients with illnesses, injuries, and chronic diseases. The role includes patient assessments, care documentation, physician reporting, education of patients and families, coordination of discharge, and oversight of LPNs, CNAs, and aides. Candidates need an active, unrestricted Michigan RN license, at least one year of clinical RN experience, CPR certification, reliable transportation, and a valid driver's license and car insurance.
