The Elizabeth Hospice is hiring an RN Case Manager to provide professional nursing care to adult, pediatric, and perinatal hospice patients. The role develops and coordinates plans of care, supervises LVNs and home health aides, documents care, provides patient and family education, and supports 24-hour coverage through on-call shifts. The position is based in hospice office locations across San Diego County and south Riverside County, requires travel for patient visits, and pays $45.00-$61.00 per hour based on RN licensure.
