Description
The Elizabeth Hospice is hiring an RN Case Manager to provide professional nursing care to adult, pediatric, and perinatal hospice patients. The role develops and evaluates plans of care, coordinates with interdisciplinary teams and providers, directs and supervises LVNs and home health aides, documents care, provides patient and family education, and participates in on-call coverage. The position is based in North San Diego County and South Riverside County, requires travel for patient visits, and pays $45.00-$61.00 per hour based on RN licensure.
