Description

VHS Hospice is seeking a compassionate and experienced Hospice RN Case Manager to coordinate and manage the care of hospice patients. This role involves developing individualized nursing care plans, educating patients and families, supervising LPNs and Hospice Aides, monitoring patient status, coordinating discharge planning, and maintaining accurate clinical documentation. The ideal candidate will have a current Virginia RN license, previous case manager experience, and demonstrated clinical expertise.

Additional Information

Required Previous Experiences: Previous experience as a Registered Nurse.
Previous Case Manager experience in a healthcare setting.
Discharge planning experience.

Excellent verbal and written communication skills with the ability to educate patients, families, and staff.
Ability to collaborate effectively with interdisciplinary team members, physicians, patients, families, and community partners.
