Description
Lighthouse Hospice is hiring a Hospice Transitional Care RN to coordinate, assess, and admit hospice-eligible patients from hospitals, facilities, physician offices, and community settings. The role conducts clinical evaluations, reviews medical records, educates patients and families, supports discharge planning and care coordination, maintains EMR documentation, and collaborates with physicians, referral sources, and interdisciplinary teams. The position serves North Team locations in Northbrook, Buffalo Grove, Des Plaines, Glenview, Skokie, and Arlington Heights and requires an RN license, at least two years of relevant nursing experience, CPR certification, and reliable transportation.
