Community Physicians is hiring a full-time Hospital Case Manager (Licensed Social Worker) for its Transitions of Care Department in Winfield, Illinois. The role coordinates discharge planning and post-acute care for medically complex older adults, conducts psychosocial assessments, advocates for patients and families, supports caregivers, and maintains EMR documentation. The position requires a BSW or MSW, an active Illinois LSW or LCSW license, and 3–5 years of relevant experience; knowledge of Medicare/Medicaid guidelines and DuPage County resources is preferred. The schedule is Monday–Friday with occasional weekend flexibility, and the salary is $72,000–$80,000 annually.
