This dual-role position involves blending mental health and housing stability to support individuals in Montgomery County, PA towards lasting stability and wellness. Responsibilities include meeting with clients, developing and updating Housing Stability Plans, conducting home visits, connecting individuals to resources, ensuring compliance with HUD requirements, and participating in case reviews.
