Description
The DHS Housing Case Manager provides comprehensive case management to individuals and families experiencing homelessness or housing instability in shelter settings. The role develops individualized housing plans, connects clients to resources, promotes housing stability and long-term self-sufficiency, and collaborates with clients, service providers, landlords, community agencies, and the social service team. A bachelor's degree in a relevant field and at least two years of case management or housing services experience are required; experience with vulnerable populations is preferred.
