Description

The Department of Mental Health is hiring a Human Service Coordinator Transitional Age Youth for its Essex North Site Office. The role provides assessment, outreach, service coordination, person-centered advocacy, individual service-plan development and monitoring, crisis support, referrals, and community-network support for DMH-eligible transitional-age individuals. It is a full-time 37.5-hour position Monday through Friday, with Saturday and Sunday off, and requires at least three years of relevant human-services or social-work experience or an equivalent combination of experience and substitutions.
