Description
Vynca is hiring a hybrid LVN Case Manager for its Enhanced Care Management team in Orange County, California. The role serves as the client’s primary care contact, coordinates providers and benefits, develops care plans, advocates for members, connects clients to social and community services, documents progress in EMR and CRM systems, and completes reporting. The position requires an active California LVN license, at least two years of relevant care-management or vulnerable-population experience, a valid driver’s license, reliable transportation, and availability Monday through Friday; travel throughout the Orange County area may occur up to five days per week, and candidates must reside within 20 miles of the assigned territory.
