Description
The Case Manager provides case management, crisis intervention, discharge planning, group and individual work, mental health and substance use disorder counseling, screening, referrals, coordination with physical health providers, placement support, transportation, and field-based services. The role requires accurate documentation, confidentiality, program participation, and collaboration with ACT and ICM teams, with possible on-call and shift coverage. Qualifications include a master's degree or a bachelor's degree with four years of social-services experience, relevant housing or SUD experience, cultural competency, a valid California driver's license, and current auto insurance; bilingual ability, Microsoft Office, and EHR software knowledge are preferred.
