Description
The ICMS Case Manager provides coordinated case management and supportive services to formerly homeless individuals with special needs in a multi-unit permanent supportive housing complex. The role conducts housing intake and needs assessments, develops Individual Service Plans, coordinates with property management and community providers, links clients to employment, education, health, counseling, and benefits services, resolves tenant issues, maintains client records, and prepares transition reports. It requires 1–2 years of relevant experience or a related bachelor's degree, experience with youth experiencing homelessness, foster care, probation, or mental health systems, Microsoft Office proficiency, reliable transportation, and a five-day office-based schedule. The salary is $50,000–$55,000 annually, with medical, dental, vision, retirement, and other benefits.
