Description
The ICMS Case Manager provides coordinated case management and supportive services to formerly homeless individuals with special needs in a multi-unit permanent supportive housing complex. The role conducts housing intake and needs assessments, develops Individual Service Plans, coordinates with property management, mental health providers, and community agencies, links clients to employment, education, benefits, and other services, resolves tenant issues, maintains client records, and prepares transition reports. The position requires 1–2 years of relevant experience or a related bachelor's degree, experience with youth experiencing homelessness or related systems, reliable transportation, Microsoft Office proficiency, and office-based work five days per week.
