Description
The Service Coordinator provides service coordination for adults and children in HHSC Waiver and General Revenue programs, primarily in Taylor County and across the Center’s five-county coverage area. The role conducts outcome planning, frequent in-person contact with consumers, families, and service providers, assesses needs, monitors service delivery, documents care, coordinates community resources, and advocates for consumers. It is a full-time position reporting to the Director of IDD Authority Services, with Monday–Friday 8 a.m.–5 p.m. flexible hours, benefits, and eligibility for one- and two-year increases.
