Description
Amara Hospice Care is seeking an Intake Coordinator to manage the hospice referral and intake process, serve as the initial contact for prospective patients and families, collect and document referral information, coordinate resources, track census and levels of care, follow up on consent and admission documentation, communicate status updates to internal leaders and referral sources, and perform clerical office duties while maintaining confidentiality and supporting quality improvement efforts.
