Heart to Heart Hospice is hiring a full-time Intake Coordinator to manage patient referrals and admissions, initiate and maintain EMR workflows, track census and care-plan activities, ensure regulatory and contractual compliance, build referral relationships, report referral trends, and support quality improvement and community-resource activities. The role requires 3–5 years of healthcare experience, admissions or customer-service experience, EMR software experience, a valid driver’s license and automobile insurance, and knowledge of CHAP standards and private-payer practices; a healthcare or related degree is preferred.
