Description
The Care Coach provides proactive, patient-centered care coordination and social needs support for high-risk senior patients, serving as a primary contact and coordinating care across primary care, specialists, pharmacies, home health, and community providers. The role includes clinical screening, home visits, chronic disease education, post-hospital and emergency-department follow-up, care transitions, and connection to community resources. It requires at least three years of ambulatory, primary care, or senior-care experience with direct patient care, bilingual English-Spanish ability, and experience with high-risk or geriatric populations. The position is mobile, requires travel to patients’ homes and community settings, and offers a full-time schedule with a stated annual base pay range of $53,700 to $72,600.
