Humana is hiring a Care Coach to provide proactive, patient-centered care coordination and social needs support for high-risk senior patients. The role serves as a primary contact, conducts clinical screenings, home visits, chronic disease education, post-hospital and emergency-department follow-up, and coordinates care across providers and community resources. It requires 3+ years of ambulatory, primary care, or senior-care experience with direct patient care and involves a combination of clinic-based and field work, including travel to patients’ homes and healthcare facilities. The position offers a $2,500 sign-on bonus, medical, dental, and vision benefits, and a starting base pay range of $53,700 to $72,600 per year.
