Description
Humana is hiring a Care Coach to provide proactive, patient-centered care coordination and social needs support for high-risk senior patients. The role serves as a primary contact, conducts clinical screenings and home visits, supports chronic disease management, coordinates care across providers and community resources, and follows up after hospitalizations or emergency department visits. It requires bilingual English-Spanish ability, at least three years of relevant healthcare experience, and a combination of clinic-based and field work. The position is scheduled Monday through Friday, 8:00 AM to 5:00 PM, with an average of two days per week in the center and two days per week in the home, and pays $53,700 to $72,600 annually.
