Clarvida is hiring an Intensive Case Manager in Augusta, Georgia. The role provides community-based care coordination by developing and implementing Individual Recovery Plans, delivering support and training, connecting clients with community services, monitoring progress through regular visits, and coordinating wraparound care with doctors, residential providers, and other service organizations. The position is full-time, weekday-based, and requires a bachelor's degree in psychology, social work, or a related field, strong communication and cultural sensitivity, a valid Georgia driver's license with a clean motor vehicle record, and a clean criminal background check.
