Description
The Community Habilitation Specialist provides assessment, training, and assistance to developmentally disabled individuals enrolled in the HCBS Waiver program and living in a home situation. The role works with consumers in their homes or community based on their Community Habilitation Plan, supports independence, individualization, productivity, and community inclusion, and follows hours determined by the consumer’s Individualized Service Plan. The specialist must communicate with supervisors, consumers, and families about schedule changes, absences, and other issues, and must maintain monthly telephone communication with supervisors regarding consumer care, time off, job performance, and authorization matters.
