Description
Rising Ground is hiring a full-time Case Planner to provide youth-centered case management for youth in Non-Secure Placement or Limited-Secure Placement facilities and their families. The role develops service and discharge plans, coordinates interdisciplinary teams and community providers, conducts family visits and home assessments, documents progress in Connections, attends court hearings, and supports aftercare. It is based at an NSP or LSP facility, requires travel throughout New York City and surrounding areas, and includes day, evening, occasional weekend, and on-call hours.
