Description
Vynca is hiring a remote Lead Care Manager to coordinate person-centered care for vulnerable populations. The role assesses physical, mental, behavioral, social, and housing needs; develops care plans; connects clients with healthcare and community services; advocates with providers; documents progress in EMR and CRM systems; and completes reporting. It requires at least two years of experience as a care manager, care navigator, or community health worker, a valid driver's license, reliable transportation, and availability Monday through Friday from 8:30 a.m. to 5:00 p.m. Pacific Time, with flexibility for evenings and weekends. English/Spanish bilingual ability is preferred, and applicants are limited to the listed states.
