Description
The Lead Care Manager develops personalized care plans, coordinates member services, assesses medical and social needs, provides education and emotional support, advocates for members, arranges appointments and treatment plans, and maintains telehealth and in-person contact. The role serves diverse populations in the Enhanced Care Management program, including individuals experiencing homelessness, those at risk for avoidable hospital use, people with serious mental health or substance use needs, justice-involved adults, and others. Candidates must be bilingual in Spanish and English, have a high school diploma or GED, at least one year of relevant experience, a valid California driver's license and vehicle insurance, and the ability to travel by personal vehicle. The position pays $25–$28 USD and offers medical, dental, and vision benefits after 60 days of employment.
