Description
The SSVF Lead Case Manager is a full-time, salaried, exempt staff-level role supporting veterans and families seeking permanent housing. The position maintains an assigned caseload while providing coaching, case consultation, workflow support, documentation review, and onboarding assistance to Case Managers. It also coordinates housing resources, community and VA partners, referrals, home visits, HMIS documentation, budgets, temporary financial assistance, and outreach. The role is based at InteCare’s Indianapolis offices, requires significant community travel, and allows telecommuting when approved.
