Human Touch Home Health is hiring a Licensed Master Social Worker for PRN home health opportunities throughout Northern Virginia. The role involves completing psychosocial assessments, identifying barriers to care, connecting patients and families with community resources, providing counseling and emotional support, assisting with care and discharge planning, advocating for patient needs, and documenting in WellSky. Applicants need a master's degree in social work, Virginia social work licensure, strong assessment and communication skills, knowledge of community resources, a valid driver's license, and reliable transportation; home health or healthcare social work experience and WellSky experience are preferred but not required.
