Description
Beacon is hiring a full-time Licensed Practical Nurse to partner with the Director of Nursing and coordinate healthcare for individuals with intellectual and developmental disabilities, severe mental illness, autism, and high-acuity behaviors. The role involves visiting assigned homes, monitoring resident health, administering medications and vaccinations, completing assessments, coordinating medical care and appointments, triaging concerns, auditing EHR and EMAR use, training staff, supporting infection control, and participating in interdisciplinary care teams and medical on-call rotation. The position is Monday through Friday, 8:00 a.m. to 5:00 p.m., and requires a valid LPN license, driver’s license, English proficiency, background check, and familiarity with nursing, healthcare terminology, communication, de-escalation, computer, and medical information systems.
