Description
CareHarmony is looking for an experienced, fully remote Licensed Practical Nurse (LPN/LVN) to manage patient care, resolve gaps in clinical and non-clinical care, identify and coordinate community resources, educate patients on chronic condition management, perform medication management, ensure timely service delivery, resolve patient questions, and assist with referrals and appointments. The role requires strong communication skills, technical aptitude with Microsoft Office Suite, and an active Multi-State/Compact License.
