Description
The Licensed Practical Nurse provides skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse, including vital signs, medication administration, wound care, patient education, and monitoring of clinical changes. The role also supports clinical documentation, physician-order tracking, referrals, scheduling, electronic medical records, care coordination, and quality-assurance activities. Applicants must hold a current and unrestricted LPN license, graduate from an accredited practical nursing program, have current CPR/BLS certification, and meet background, health, and competency requirements; one year of nursing experience is preferred, along with home-health, wound-care, medication-management, and electronic medical record experience.
