Blue Ridge Care is hiring a Licensed Practical Nurse (LPN) for its PACE program in a hybrid clinic and home-care role. The LPN provides direct participant care, supports care coordination, educates participants and families, documents care in an electronic health record, participates in interdisciplinary meetings, and works on an on-call rotation. The position requires a current LPN license, at least one year of experience with the elderly population, CPR/BLS certification, a valid driver’s license, reliable transportation, and EHR competency.
