MFM Health is hiring a full-time Licensed Practical Nurse to serve as a Provider Support and Clinical Coordinator in its primary care team in Danvers, Massachusetts. The role supports physicians and advanced practice providers through clinical workflow coordination, EMR documentation, chart preparation, medication and referral management, test-result routing, and continuity-of-care coordination. The LPN will also provide coverage for medical assistants and nursing staff as needed and collaborate with multidisciplinary healthcare teams. A current Massachusetts LPN license is required, while primary care, family medicine, ambulatory care, and Athenahealth experience are preferred. The pay range is $34–$40 per hour, with benefits including paid time off and a 401(k) employer match.
