TriHealth is hiring a full-time Licensed Practical Nurse for its physician-office Medical Home program in Blue Ash, Ohio. The role provides direct and indirect primary-care patient care, including rooming, clinical procedures, immunizations, venipuncture, point-of-care testing, retinal imaging, documentation, patient outreach, care coordination, and chronic-disease education. The position requires an approved healthcare technical or vocational program, BLS and CPR, medical-office flow, EMR knowledge, and 3–4 years of clinical healthcare experience; physician-practice experience is preferred.
