Description
Vitra Health is hiring three Licensed Practical Nurses (LPNs) to provide nonclinical support for aging adults, people with disabilities, and individuals with complex care needs. The role includes intake and assessment visits, in-home caregiver education, psychosocial and environmental assessments, care-plan participation, discharge planning, community-service navigation, documentation, and multidisciplinary collaboration. Candidates must be licensed by the Massachusetts Board of Registration in Nursing, have at least two years of recent direct-care experience with elders or adults with disabilities, hold a valid driver's license with proof of insurance, and demonstrate strong communication skills; bilingual ability and relevant healthcare experience are preferred.
